This program has demonstrated effectiveness in the case of fraility and cognitive impairment allowing our clients to remain in their familiar surroundings. Additional monitoring equipment can be installed for conditions such as diabeties, BPs and  oxygen saturation, monitored by our centralised clinical team that actively keeps clients stable and avoids unnecessary hospital admissions.

Hospital in the Home style services are also available as an additional service to clients whose needs are greater than those able to be supported with CACP. These services can be of short duration and aim to aviod unnecessary or prolonged hospital admissions.
