Palliative Care
Palliative Care aims to provide quality care at the end of life, in support of those living with a terminal illness, their family and caregivers.
Care provided by specialist practitioners, is ideally based in ‘advanced care planning’ in which the individual formulates and communicates their ‘personal’ desires and preferences for a quality end of life. In addition to meeting the needs of the individual, palliative care seeks to address the needs of family and caregivers.
Envigor staff support the individual and the family to manage the physical (pain and other symptoms), emotional, social, cultural and spiritual aspects of terminal illness.
Our Case Management & Care Coordination approach provides support throughout the end of life experience in hospital, the home or residential care facility through:
- Managing complex needs of the individual and/or family unit
- Medication and Pain Management
- Personal Care
- Assistance with Meals
- Home Care
- Carer relief / respite
- Care / Family Training – mobilising and personal care
- Transport to GP, specialist and hospital as required
