When you’re aged, disabled or acutely infirmed, when maintaining your current living arrangement is simply not an option, when you don’t know what services are available to you, how to access them, or even where to start – thankfully your Case Manager is working for YOU.

At Envigor your Case Management service is focused on protecting YOUR personal choice, independence, safety and security and aims to provide a ‘whole of person’ approach which is tailored to individual circumstance and:

• provides comprehensive personalised assessment, advice and assistance
• ensures all available options are assessed by an experienced health care profession
• recommends the most suitable options for your personal circumstances
• navigates the complexities of the health, aged and community care systems on your behalf
• optimises your independence, choice and security by provided ongoing reassessment, monitoring, evaluation and management of care planning and provision

The goal of Case Management is to ensure that at all times your personal Case Manager (a qualified and experienced healthcare professional) is representing YOU the individual, your wishes and best interests.

Envigor Case Management provides two service options

1. Health Professional Advocacy and support ongoing; ensuring professional representation and maintenance of personal choice, independence, safety and security – third party engagement, management and quality monitoring
   
2. Acute Intervention engagement at the time of major life change; to navigate the individual, family or carer through the complexities of acute hospital, health, aged and community care systems, identifying and accessing the full range of services as may be required, from in-home support to Residential High Care facility

As Health Professional Advocate, your Case Manager will actively ensure:

• Regular personalised needs reassessment
• Maintenance of strong professional communication with general practitioners, medical specialists, other health professionals and care service providers to ensure the best possible, personalised outcome for the individual
• Discussion of financial considerations and implications of service options

Whether you require high level specialised nursing care, or lower levels in-home assistance your personal Case Manager will be there for you.
